TO REDEEM: Enclose the following in a stamped envelope:

Original on-pack booklet with all requested information filled in. (download booklet)
Original sales receipt* with purchase price and date circled (no photocopies). The date on the receipt must be between 11/1/07 and 7/31/08. *Keep a copy of your sales receipt for warranty purposes.
“Proof of Purchase”- cut off of the original box. (Some mail order businesses do not include JOI packaging in shipments so you may not receive a UPC. For those businesses, an invoice/packing list clearly showing matching UPC/SKU number will suffice).
